muscle or circulation trouble?

Question

I am 41, 20 lbs. overweight and have a very physical job. I work out 3 times per week faithfully. My knees and lower extrmities continuously ache even with ibuprophen. I take no regular meds. This has been going on for well over 10 years and it's getting worse. What should I do?
